Health Form Submission Process<\/h3>\n

To be cleared for participation, all athletes must complete and submit their health form using the steps below:<\/p>\n

  1. Print the Health Form PDF<\/strong>
    Download and print the official health form paperwork.<\/li>\n
  2. Complete All Required Sections<\/strong>
    Fill out the form fully and ensure it is signed where required.<\/li>\n
  3. Scan and Email the Completed Form<\/strong>
    Scan the completed paperwork and email it to     joshoswaltpsfca@gmail.com<\/a>.<\/li>\n<\/ol>\n

    Forms must be submitted before participation is permitted.<\/p>\n
