2023 Big 33 Cheerleader Information

2023 Big 33 Pre-Participation Physical Evaluation (Health Form): This form needs to be completed by you and signed by a parent/guardian if you are under 18. Please provide a copy of your high school sports physical form (PIAA CIPPE form, include section 8 as well, if completed). The physical forms need to be dated on or after June 1, 2022. Send a copy of the front and back of your insurance card as well. (Deadline Date: April 7, 2023)

2023 PSFCA Athletic Participation Release Form: This form needs to be completed by you and signed by a parent/guardian if you are under 18. (Deadline Date: April 1, 2023)

2023 Program Ad & Sponsor Checklist Form: Complete and return to us indicating payment(s) & what size ad you created online (Deadline Date: April 22, 2023)
