Health Form Submission Process
To be cleared for participation, all athletes must complete and submit the required health paperwork by following the steps below:
- Download and Print the official Health Form PDF.
- Complete All Sections and obtain all required signatures.
- Include Insurance Card – A clear photo or scan of the front and back of the athlete’s insurance card is required.
- Submit by Email – Scan and email the completed health form and insurance card images to: jbrubaker@big33.org.
All documents must be submitted and approved prior to participation.
