2026 PSFCA Athletic Participation Release Form: This form needs to be completed by you and signed by a parent/guardian if you are under 18. (Deadline Date: April 1, 2026)

Health Form Submission Process
To be cleared for participation, all athletes must complete and submit their health form using the steps below:
- Print the Health Form PDF
Download and print the official health form paperwork. - Complete All Required Sections
Fill out the form fully and ensure it is signed where required. - Scan and Email the Completed Form
Scan the completed paperwork and email it to joshoswaltpsfca@gmail.com.
Forms must be submitted before participation is permitted.
