2025 Health Form: Complete entire PIAA CIPPE Form (excluding wrestling section). (Deadline Date: April 1, 2025)
See Athletic Director, Athletic Trainer or head coach for physical packet from this school year. Send scan/picture of entire CIPPE form from this past fall.

2025 PSFCA Athletic Participation Release Form: This form needs to be completed by you and signed by a parent/guardian if you are under 18. (Deadline Date: April 1, 2025)
