2024 PSFCA Athletic Participation Release Form: This form needs to be completed by you and signed by a parent/guardian if you are under 18. (Deadline Date: April 1, 2024)

2024 PSFCA Pre-Participation Physical Evaluation (Health Form): This form needs to be completed by you and signed by a parent/guardian if you are under 18. A complete physical will be provided by Big 33 when you arrive at your first practice. (Deadline Date: April 1, 2024)
